A "tri suit" is one choice of garment you can wear through the entire race. This helps make the suit more comfortable for swimming. After the swim, the thin pad dries out quickly and doesn't feel like a big, wet diaper on the bike ride.
The answer is no. You should not wear underwear (except a sports bra) under your trisuit in a race. What to look for: Race number belts - the name says it all, a race number belt is just an adjustable belt that displays your race number during the run. This piece of gear is pretty nice to have during a triathlon because you don't need to worry about changing clothes or riding with your number on.
Some women can get away with a tight triathlon suit, while others are more comfortable in a sporty swimsuit top or a moisture-wicking sports bra that will not get saturated during the swim. Make sure all your attire, including your sports bra, is 'tried and true' come race day.
A good pair of tri shorts provides some level of bike padding, but at a level that doesn't impede the run at all. A bike seat pad that is sufficient for longer rides and quick-drying as you exit the water. Bike pads in triathlon shorts are not as substantial or thick as those you find in single-purpose bike shorts.

Clothing for a Triathlon
You can opt to change into clothes specific to each leg of the race—or not. Some triathletes choose to do the whole race in a swimsuit for quicker transitions. Other swimmers simply pull on a pair of shorts before jumping on their bike. In general, tri clothing should fit snugly.Headphones, headsets and audio devices are not allowed at any time during any triathlon event. Most races allow you to carry your cell phone if you wish, but it must be stored out of sight in a bike bag or jersey pocket.
DO YOU HAVE TO WEAR A SWIM CAP IN A TRIATHLON? If a swim cap is provided by the race organizer, which is the case in most triathlons, it is mandatory to wear it. Altering it in any way may result in a penalty up to disqualification.

Higher-end wetsuits tend to be thinner, as they are designed with more elite athletes in mind. An entry level suit will often provide greater warmth and buoyancy but reduced flexibility. Maximum wetsuit thickness for competitive triathlons is 5mm to help reduce the risk of overheating.
To determine fit across the chest, while wearing the suit, cross the arms in front to check neoprene tightness between the pectoral and the shoulders. Many wrinkles means the neoprene is loose, and the suit is probably too big. Try one size under. If the arms bind and the chest feels compressed, go one size up.
Many male triathletes wear tri-shorts for all sections of the race -- these are similar to bike shorts, but have a fast-drying pad in the crotch area so you dry off faster during the cycle ride. In some races, you will be either allowed or required to wear a wetsuit during the open-water swim.

For most triathlon races the answer to whether or not you should wear a wetsuit is a YES! If it's allowed, you should wear your triathlon wetsuit. If you're comfortable wearing it, then it will only help your swim be faster and more comfortable.
